Do I need to provide ID?

It depends on your situation.

If your clinic recently joined Tend, you don’t need to upload ID, we already have your information securely on file. We’ll simply match your medical records.

If you’re a new patient to Tend, you’ll need to upload a photo of your ID for enrolment verification.

If we’ve reminded you that your enrolment is expiring and you want to stay enrolled, we may ask for ID if we don’t have it on file.
